I know of no guide. The death sounds are played when a soldat is regularly fragged (i.e., "not regularly" would be by explosives).

The game chooses them at random.

Yes, one of those three is played at random upon any death, so no one is really played more often than another.

Not any death; explosions trigger bryzg.WAV and some others, and deaths by FlameGod or Lava polygons use a different death cry.
